Key Responsibilities:- Welcoming patients into our continuous care programs and reviewing benefits and services included
- Coordinating with patients' Respiratory Therapists and pulmonary providers to ensure a collaborative approach to care
- Educating patients on the frequency and use of their assigned in-home monitoring devices, if applicable
- Creating personalized, comprehensive care plans with patients via phone or video visits
- Identifying and addressing any barriers to patient success
- Providing specific education and coaching on patients' chronic pulmonary conditions
- Connecting with patients frequently to review readings and update their plan of care monthly
- Serving as patients' first contact for all non-emergent needs, such as medication refills, scheduling, and appointment reminders
- Assisting patients in navigating their healthcare, including scheduling appointments, coordinating with specialists, and ensuring preventative screenings are completed
- Reviewing and evaluating in-home device readings in real-time, during normal business hours
- Following established protocols for identifying, communicating, and documenting device trends and associated symptoms in patients' medical records
- Escalating concerning readings and/or symptoms to providers following a communication protocol established by the provider
- Establishing meaningful rapport that builds trust, open communication, and motivation to make a positive change in patients' health
- Active and unrestricted Georgia RN license (Compact license preferred)
- At least three years of experience in adult health, preferably in adult chronic health conditions (Pulmonology, Cardiology, Care Management)
- Variety of Electronic Medical Record (EMR) experience
- Proficient knowledge, skill, and interest in basic computer skills
- Proficient in problem-solving and ability to multi-task
- Excellent communication skills (oral and written)
- Excellent teamwork skills
- Clean background check and drug screening
- Comfortable working remotely but collaboratively
