ROLE SUMMARY:

The Substance Abuse Recovery Specialist plays a crucial role in delivering comprehensive assessments, counseling, crisis intervention, and follow-up services for individuals facing substance use challenges within our Permanent Supportive Housing framework. Collaborating with a multidisciplinary team, this position engages with various community organizations to facilitate both inpatient and outpatient substance use treatment.

KEY RESPONSIBILITIES:

  • Conduct outreach to individuals with a history of homelessness residing in permanent supportive housing.
  • Encourage residents to participate in services and activities that enhance their well-being by establishing trust and collaborating with the Integrated Care Management Services (ICMS) team.
  • Build partnerships with substance use treatment providers and streamline the referral process; maintain an updated resource directory of substance use disorder services.
  • Work alongside the Assistant Director and other staff to design and implement programming across all Permanent Supportive Housing sites.
  • Assess and identify individuals exhibiting signs of substance use disorders; collaborate with them to create tailored treatment plans using a harm reduction framework.
  • Provide ongoing support to ensure successful completion of referrals; advocate for clients as necessary, engaging in care coordination and discharge planning.
  • Share expertise on substance use disorders with outreach team members to enhance service delivery.
  • Maintain accurate documentation in both written and electronic formats.
  • Assist in coordinating departmental activities as directed by the supervisor.
  • Participate in regular staff, agency, and community meetings as required.

QUALIFICATIONS:

  • Proven experience in field-based services, with a strong understanding of the Coordinated Entry System (CES) aimed at achieving functional zero for homelessness.
  • In-depth knowledge of challenges faced by homeless individuals, including chronic health issues, substance abuse, mental health concerns, and resources for underserved populations.
  • Demonstrated empathy and a non-judgmental approach when working with individuals experiencing homelessness.
  • High tolerance and understanding for clients with diverse mental and physical health needs.
  • Familiarity with harm reduction and housing-first philosophies.
  • Comprehensive understanding of various substance use disorders as classified by DSM-5.
  • Proficient in extensive documentation, electronic data entry, and record-keeping.
  • Clinical experience working with individuals facing multiple diagnoses, including substance use and mental health disorders.

REQUIREMENTS:

  • Active CADAC, CAADE, or CAPP accreditation is mandatory.
  • A minimum of one year of experience working with homeless individuals or at least two years of experience in direct health, behavioral health, or substance use counseling.
