Job Overview

MPAC Healthcare is seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team in Norfolk, VA. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to seniors in a dynamic and supportive environment.

Key Responsibilities
  • Provide initial assessments, psychotherapy, and ongoing support to residents and their families
  • Collaborate with interdisciplinary teams to ensure patients receive comprehensive care
  • Document patient needs, progress, and outcomes in accordance with regulatory requirements
  • Develop and implement individualized care plans to address mental health issues and promote overall well-being
  • Communicate effectively with facility staff, families, and other healthcare professionals to ensure seamless care coordination
Requirements
  • Master's degree in Social Work and valid LCSW license in the state of Virginia
  • At least two years of experience in counseling and therapy, preferably in a healthcare setting
  • Strong knowledge of mental health diagnoses, therapeutic approaches, and documentation requirements
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work autonomously and as part of a team
