Division Overview: The Corrections Transition Programs (CTP) serve as a comprehensive network of clinical reentry management services aimed at enhancing successful reintegration outcomes. CTP offers tailored case management services designed to assist individuals in their transition back to families and communities following incarceration. Our efforts extend both within correctional facilities and in the community, providing essential reentry support, including behavioral health referrals, public benefits enrollment, employment and housing assistance, and obtaining vital records.

Position Summary: We are currently seeking a part-time Case Manager to deliver addiction intervention, AODA counseling, and recovery support services to clients and their families. This role is pivotal in coordinating services, agencies, resources, and individuals to assist clients in reaching their established objectives. The position may involve working with high-risk caseloads, co-occurring clients, families, and children.

Key Responsibilities:

  • Facilitate individuals in achieving their service plan goals through direct services and coordination with behavioral health, public agencies, and community resources.
  • Assess, evaluate, and monitor individual progress in relation to established client-centered service plans and parole conditions.
  • Complete intake documentation for program enrollment and participation.
  • Provide referrals and crisis intervention tailored to the identified needs of individuals.
  • Conduct interviews, review records, and collaborate with other professionals to evaluate individual progress and suitability for program participation.
  • Engage with internal and external partners, participate in staff meetings, and contribute to integrated staffing for individuals receiving TASC services.
  • Identify and refer individuals with behavioral health needs.
  • Prepare and submit internal and external program reports.
  • Maintain comprehensive documentation related to individual and program activities, including screenings, enrollments, service plans, progress notes, and discharge summaries.
  • Monitor individual progress towards service goals.
  • Assist clients with enrollment in public benefit systems and obtaining vital records.
  • Oversee compliance with program requirements through monitoring of TASC and external database systems.
  • Foster effective working relationships with internal and external partners.
  • Participate in meetings to review and adjust client-centered service plans as necessary.
  • Obtain and analyze progress reports from both internal and external sources.

Qualifications:

  • Bachelor's degree preferred in human services, social work, psychology, or related fields.
  • One or more years of experience in human service activities, or an equivalent combination of training and experience.
  • Understanding of human behavior, assessment techniques, and signs of substance abuse disorders, particularly with special populations.
  • Familiarity with the Code of Ethics for substance abuse professionals.
  • Knowledge of federal drug and alcohol confidentiality laws (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act.
  • Awareness of the criminal justice system.
  • Must possess a valid driver's license, current auto insurance, and reliable transportation to meet program and client needs.
