Elderly Guardianship Case Manager

Role Summary: This position involves supervising and delivering therapeutic interventions to a targeted demographic of elderly individuals and other at-risk groups requiring assistance within the designated service area. This role is part-time, offering up to 18 hours of work per week.
Key Responsibilities:
- Provide direct support to clients enrolled in the Elderly Guardianship initiative, including those with developmental challenges and mental health issues.
- Be available for 'on-call' support for clients participating in the Elderly Guardianship program.
- Collaborate with the Eldercare Options Coordinator to facilitate client intake and assist with legal processes as required.
- Maintain accurate and timely records in accordance with funding requirements and organizational standards.
- Conduct assessments and provide referrals tailored to the specific needs of elderly clients.
- Perform monthly client visits to ensure ongoing support and engagement.
- Participate in Care Plans, annual evaluations, and other relevant meetings for clients.
- Work in partnership with healthcare providers, nursing facilities, legal representatives, and funeral services as necessary.
- Act as the agency's liaison with other community organizations focused on elderly services, as directed by management.
- Collaborate effectively with supervisors and administrative personnel to enhance or develop the Elderly Guardianship program.
- Engage in required training to stay informed about current practices in elderly services.
- Report any suspected cases of abuse or neglect to the appropriate authorities as outlined in procedures.
Qualifications:
- Bachelor's degree in Social Work or a related human services discipline.
- 3-5 years of relevant experience in direct service roles within social work or human services.
- Ability to follow instructions and apply them responsibly, utilizing unsupervised time effectively.
- Demonstrated sensitivity and respect for diverse populations, particularly those facing socio-economic, emotional, physical, and spiritual challenges, with a commitment to teamwork and the agency's mission.
- Possession of a reliable vehicle, valid driver's license, and current auto insurance compliant with state and federal regulations.
- Exhibit maturity, flexibility, reliability, and strong organizational capabilities.
