Position Overview

Role Summary: The individual in this role will manage and deliver therapeutic interventions to a targeted demographic of elderly individuals and other at-risk groups within the designated service area. This is a part-time role, requiring up to 18 hours of work each week.

Key Responsibilities:

  • Provide direct support to clients enrolled in the Elderly Guardianship program, including those with developmental challenges and mental health issues.
  • Be available for 'on-call' assistance for clients participating in the Elderly Guardianship program.
  • Collaborate with the Eldercare Options Coordinator to facilitate client intake and assist with legal processes as required.
  • Maintain accurate and timely records in accordance with funding requirements and accreditation standards.
  • Conduct assessments and offer referral services tailored to the unique needs of elderly clients.
  • Perform monthly client visits to ensure ongoing support and engagement.
  • Participate in Care Plans, annual reviews, and other relevant meetings concerning client welfare.
  • Work in partnership with healthcare providers, nursing facilities, legal representatives, and funeral services as necessary.
  • Act as a representative of the agency in collaborations with other community organizations focused on elderly care, as directed by management.
  • Collaborate with supervisors and administrative personnel to enhance and develop the Elderly Guardianship program.
  • Engage in essential training to stay informed about current practices in elderly services.
  • Report any suspected cases of abuse or neglect to the appropriate authorities as outlined in procedures.

Qualifications Required:

  • A Bachelor's degree in Social Work or a related human services discipline.
  • 3-5 years of hands-on experience in a relevant field, such as social work or human services.
  • Ability to follow instructions and apply them effectively, demonstrating initiative during unsupervised periods.
  • Demonstrated sensitivity and respect for diverse populations, particularly those who may be marginalized due to socio-economic, emotional, physical, or spiritual factors, along with a capacity to work within a team-oriented environment.
  • Possession of a reliable vehicle, valid driver's license, and current auto insurance compliant with state and federal regulations.
  • Exemplary maturity, adaptability, reliability, and strong organizational capabilities.
