Role Overview
The Substance Use Patient Care Coordinator plays a crucial role in assisting individuals and their families as they navigate the complexities of the healthcare system. By integrating personal interaction with technological resources, the coordinator ensures a smooth transition of care, with continuous monitoring and follow-up on care plans. This role is essential in alleviating the stress and confusion often faced by patients and their support networks through effective communication and coordination.

Key Responsibilities

  • Serve as a Substance Use Patient Care Coordinator, engaging with patients to evaluate their readiness for treatment and connecting them with appropriate care services.
  • Employ motivational interviewing techniques to foster respectful and culturally sensitive communication with patients.
  • Stay informed about various substances, withdrawal symptoms, and treatment alternatives to provide accurate education and guidance to patients.
  • Act as a liaison to Addiction Medicine and outpatient clinics, facilitating efforts towards recovery and sobriety.
  • Participate in training and meetings as required by relevant health programs.
  • Clearly articulate the services available to patients, families, and caregivers.
  • Advocate for the expansion of medication-assisted treatment (MAT) programs within the healthcare facility.
  • Assist patients in coordinating their transitions between care settings, including managing referrals, appointments, and necessary medical equipment.
  • Maintain regular communication with care team members to enhance service delivery and improve patient outcomes.
  • Collaborate with the SUN Program Supervisor and MAT Physician Champion to support hospital initiatives and patient care pathways.
  • Ensure the confidentiality of all patient treatment efforts throughout their care journey.
  • Facilitate access to community resources that promote patient wellness.
  • Support patients in obtaining health insurance coverage.
  • Establish strong relationships with patients diagnosed with substance use disorders or co-occurring mental health conditions.
  • Document all patient interactions accurately and meet reporting requirements.
  • Provide transparent information regarding qualifications and services offered, while avoiding any clinical activities.
Compensation
Pay range between $23-$28/hr based on experience.

Qualifications
  • High school diploma or GED preferred.
  • Fully vaccinated against COVID-19 or possess an approved exemption.
  • Knowledge of substance use disorders and medication-assisted treatment.
  • Positive, nonjudgmental approach to assisting patients with substance use challenges.
  • Cultural competence in serving diverse populations.
  • Commitment to patient confidentiality and privacy.
  • Experience in healthcare settings.
  • Proficiency in medical terminology.
  • Familiarity with Google Suite, Excel, and email communication.
