Job Title: Clinical Transplant Coordinator - Uncertified
About the Role:
The RN Clinical Transplant Coordinator - Uncertified provides individualized care to patients seeking or having received a solid organ transplant. This role assumes responsibility for comprehensive care, support, and education throughout all phases of the transplant process.
Your Responsibilities:
- Participate in the multidisciplinary team to identify and meet patient needs
- Liaise between the kidney transplant program and dialysis facilities
- Utilize the nursing process to promote optimal health and well-being
- Ensure continuity of care through effective communication and collaboration
Requirements:
- Minimum two years' experience in acute care or critical care nursing
- Current Colorado Registered Nurse licensure
- Current American Heart Association Basic Life Support Health Care Provider (BLS) certification
