As a Case Management Assistant, you will play a vital role in ensuring the smooth and safe transition of patients from acute care settings to various alternative care options. These may include skilled nursing facilities, long-term acute care, inpatient rehabilitation, home infusion therapy, hospice, and home care programs.
Key Responsibilities:
- Facilitate the execution of tasks outlined in the inpatient discharge care plan, managing patient needs throughout the healthcare continuum.
- Communicate effectively with patients and collaborate with all members of the healthcare team, coordinating services with post-acute providers and medical insurance payors.
- Organize transportation and secure necessary authorizations for the Care Coordination department, while delivering regulatory notices as assigned by the Care Coordination Team.
Qualifications:
The ideal candidate will possess a proficiency level typically achieved through 2-3 years of experience in healthcare roles such as Nursing Assistant, Medical Assistant, Health Unit Coordinator, or Placement Specialist. Strong communication and customer service skills, along with effective human relation and time management abilities, are essential. Flexibility in responding to multiple demands and the ability to work weekends on a rotational basis are also required.
Compensation:
The salary range for this position is $26.074/hr - $27.430/hr.
