Job Title: Referral Coordinator

Caring Health Center is seeking a skilled Referral Coordinator to join our team. As a Referral Coordinator, you will play a critical role in ensuring timely transmission and start of services for our patients.

Key Responsibilities:
  • Schedule outside referral appointments as directed by Primary Care Physicians (PCPs)
  • Keep accurate logs of all appointments made
  • Document referral appointments in medical records
  • Send all necessary paperwork and records to specialists
  • Check referral voicemail regularly and address referral requests appropriately
Requirements:
  • Graduated from an accredited school and possess a High School diploma or G.E.D.
  • Ability to establish and maintain effective working relationships with employees, patients, and representatives of outside agencies
  • Professional mannerisms and appearance
  • Demonstrate proficiency in assessment and delivery of services to a diverse population
  • Adult, Infant, and Child CPR certification
Working Conditions:

This position requires the ability to work at a computer workstation and may involve long and arduous hours.
