Key Responsibilities- Build caring relationships with patients to promote positive health and social service outcomes.
- Improve access to health and human services for patients through in-person interactions, telephone contacts, and coordination of referrals.
- Coordinate and/or complete assessments and goals that are strength-based and solution-oriented to build treatment plans with family and service providers.
- Teach strategies for change by assessing stage of change and providing education appropriate for readiness.
- Participate in case conferences, staffing meetings, treatment teams, and other consultation meetings.
- Master's degree in a social work field, including Social Work, Marriage and Family Therapy, Psychology, and/or Mental Health Clinician.
- Licensure: Licensed or with the capability to be licensed preferred.
- One to three years of experience working with at-risk populations.
- Experience with wrap-around services required.
- Culturally Competent Interpersonal & Communication Skills.
- Comprehensive knowledge of the local community.
