Job Summary

Brightside and Mercy Medical Center, part of Trinity Health of New England, seeks a skilled Behavioral Health Specialist / Social Worker to join our team of integrated behavioral health professionals. The ideal candidate will possess a master's degree in a social work field and have experience working with at-risk populations.

Key Responsibilities
  • Build caring relationships with patients to promote positive health and social service outcomes.
  • Improve access to health and human services for patients through in-person interactions, telephone contacts, and coordination of referrals.
  • Coordinate and/or complete assessments and goals that are strength-based and solution-oriented to build treatment plans with family and service providers.
  • Teach strategies for change by assessing stage of change and providing education appropriate for readiness.
  • Participate in case conferences, staffing meetings, treatment teams, and other consultation meetings.
Requirements
  • Master's degree in a social work field, including Social Work, Marriage and Family Therapy, Psychology, and/or Mental Health Clinician.
  • Licensure: Licensed or with the capability to be licensed preferred.
  • One to three years of experience working with at-risk populations.
  • Experience with wrap-around services required.
  • Culturally Competent Interpersonal & Communication Skills.
  • Comprehensive knowledge of the local community.
