About the Role:

The Access Navigator plays a vital role in our team-based client care model at Behavioral Health Network. As a key member of our integrated behavioral health team, you will build strong relationships with clients and provide in-the-moment interventions and referrals to support their needs.

Key Responsibilities:

  • Provide evidence-based care in a collaborative team environment led by Master's-Level Clinicians.
  • Act as an advocate for clients, conducting needs assessments, case management, and direct support to access necessary services.
  • Develop a deep understanding of community resources and providers, both internal and external to BHN.

Requirements:

  • Bilingual/bicultural candidates are encouraged to apply.
  • High School diploma with 1-2 years of work experience in a related healthcare field (required).
  • Associate degree with 1-2 years of work experience in a related behavioral health field (preferred).
  • Knowledge of mental illness, the interface between mental health and medical conditions, substance abuse, and care management (required).
  • Professional demeanor and strong communication skills in interacting with clients and care team members (required).
  • Familiarity with electronic medical records (EMR), internet, word processing, and email systems (preferred).
