As a dedicated Home Health LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Your expertise and compassion will make a tangible difference in the lives of our patients and their families.
Key Responsibilities:- Provide skilled nursing services under the direction of a RN, adhering to state regulations and CenterWell Home Health policies.
- Develop and implement individualized care plans to address patients' unique needs and goals.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
- Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions.
- Document patient progress, outcomes, and any changes in their condition.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
