Key Responsibilities:- Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough assessments, identify patient needs, and provide evidence-based interventions to promote optimal health outcomes.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
- Provide education and support to patients and families on disease management, self-care, and health promotion.
- Monitor patient progress, adjust care plans as needed, and ensure timely communication with healthcare teams.
- Supervise and mentor home health aides and other healthcare professionals to ensure high-quality care delivery.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum one year of nursing experience, preferably in home health or a related field.
- Strong med-surg, ICU, ER, and acute care experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Excellent communication, organizational, and leadership skills.
