Job Summary:

As a Licensed Practical Nurse (LPN) with Eden Health, you will provide skilled nursing care to patients in their homes, working closely with our interdisciplinary team to deliver high-quality care. Your expertise will be valued as you assess patient needs, develop and implement care plans, and collaborate with physicians and other healthcare professionals to ensure seamless care transitions.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, adhering to physician-ordered plans of care.
  • Conduct regular caseload reviews with supervising RNs to ensure optimal patient care.
  • Collaborate with physicians to obtain necessary orders and coordinate care.
  • Assess and educate patients and families on diagnosis, treatment, and care plans.
  • Participate in hospice services coordination, reporting identified needs to interdisciplinary team members.
  • Utilize equipment and supplies effectively and efficiently.
  • Manage patient pain and symptoms, evaluating responses to treatments and medications.
  • Actively participate in quality management programs.
  • Document accurate and thorough visit notes in Electronic Health Records (EHRs) in a timely manner.
Requirements:
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Preferred experience: one year in an acute care setting within the last 24 months or six months in home care within the last 12 months.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
