Speech Language Pathologist Home Health Part Time

We are seeking a skilled Speech Language Pathologist to join our team at CenterWell Home Health. As a Home Health Speech Language Pathologist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
Key Responsibilities:- Evaluate and treat patients with speech, language, and swallowing disorders in a home health setting.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with our interdisciplinary team to ensure comprehensive care and coordination.
- Provide education and training to patients and their families on effective communication strategies and techniques.
- Document patient progress and maintain accurate records.
- Master's degree in Speech-Language Pathology or related field.
- Current licensure and certification in Speech-Language Pathology.
- Minimum of six months experience as a Speech-Language Pathologist in a home health setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
