Key Responsibilities:
  • Develop and implement clinical operations strategies to drive patient outcomes and satisfaction.
  • Lead and manage a team of clinicians, providing guidance, coaching, and support to ensure they have the resources and expertise needed to deliver exceptional care.
  • Conduct regular assessments and evaluations to ensure our patients are receiving the right care at the right time.
  • Collaborate with our care teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Monitor and analyze patient outcomes, identifying areas for improvement and implementing changes to drive better results.
  • Stay up-to-date on the latest clinical best practices and industry developments, sharing knowledge and expertise with our care teams to ensure they have the latest information and tools to deliver exceptional care.
  • Participate in quality improvement initiatives, working with our teams to identify areas for improvement and develop strategies to drive better outcomes.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Two years of experience as a Registered Nurse, with at least one year of management experience in a home care or hospice environment.
