As a Home Health Registered Nurse, you will:
* Provide skilled nursing visits for home health patients, including admission, case management, and follow-up care.
* Coordinate care with various healthcare personnel and report patient progress to the physician and Clinical Manager.
* Prepare appropriate medical documentation on all patients and confer with other healthcare disciplines.
**Required Experience/Skills:**
* Diploma, Associate, or Bachelor Degree in Nursing
* Minimum of one year nursing experience preferred
* Current and unrestricted Registered Nurse licensure and CPR certification
