Join Our Team of Dedicated Home Health Professionals

A Home Health Aide plays a vital role in providing high-quality care to patients in the comfort of their own homes. As a Home Health Aide, you will work under the direction of a Registered Nurse (RN) to deliver personalized care to patients with varying needs.

Key Responsibilities:
  • Assist patients with daily living activities, such as bathing, dressing, and grooming.
  • Help patients with medication management, including opening bottle caps and reading medication labels.
  • Take accurate temperature, pulse, and blood pressure measurements and report any abnormal results to the supervisor.
  • Assist patients with feeding, including safely positioning them for meals and assisting with self-feeding.
  • Provide necessary skills to assist patients with mobility, exercises, positioning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
  • Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.
Requirements:
  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • Must be in good standing on the HHA Registry (if applicable) and have completed the HHA/CNA course to work for a Medicare-certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
