Housing Stability Case Manager Job Description

Community of Hope is seeking a dedicated and compassionate Housing Stability Case Manager to join our team. As a key member of our housing department, you will play a vital role in supporting families in gaining and maintaining housing stability.

Key Responsibilities:

  • Develop and implement case management plans that incorporate goals for both adults and children related to core areas, including finding stable housing and family stabilization and support.
  • Collaborate with families and Housing Specialists to assist with housing search and placement process, including accompanying families to view units and advocating for families with landlords.
  • Network with community resources for housing, employment, emotional wellness services, training, etc.
  • Maintain regular communication with all families on caseload and document progressive engagement efforts with difficult-to-engage families.

Requirements:

  • Bachelor's degree preferred; BSW and MSW degree holders must have a LSWA/LGSW or appropriate social work license prior to starting.
  • Minimum of two years of experience working in housing/homelessness or social services field.
  • Strong knowledge of community resources or ability to become knowledgeable required.
  • Strong communication skills including verbal and writing.
