About the Role

We are seeking a passionate and dedicated Housing Stability Case Manager to join our team at Community of Hope. As a key member of our staff, you will play a vital role in providing housing stability and income-focused case management services to families and individuals in need.

Key Responsibilities:

  • Develop and implement case management plans that address the unique needs of each family or individual
  • Provide regular support and guidance to help families and individuals achieve their goals
  • Collaborate with community resources to ensure families and individuals have access to necessary services and support
  • Conduct regular assessments and evaluations to monitor progress and make adjustments as needed

Requirements:

  • Bachelor's degree in social work or related field required
  • Minimum one year work experience in social service or related field preferred
  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly preferred
