Housing Stability Case Manager Job Description

Community of Hope is seeking a dedicated Housing Stability Case Manager to join our team. As a key member of our staff, you will play a vital role in supporting families in gaining and maintaining housing stability.

Key Responsibilities:

  • Develop and implement case management plans that address the unique needs of each family.
  • Collaborate with families and Housing Specialists to facilitate the housing search and placement process.
  • Network with community resources to provide families with access to employment, emotional wellness services, and other essential support.
  • Act as a liaison with landlords and other community supports as needed.

Requirements:

  • Bachelor's Degree with a minimum of 1 year of experience in case management or a related field.
  • Strong knowledge of community resources and ability to network effectively.
  • Excellent communication and interpersonal skills.
