Housing Stability Case Manager Job Summary

Community of Hope is seeking a skilled Housing Stability Case Manager to join our team. As a Housing Stability Case Manager, you will provide housing stability and income-focused case management services to families assigned to your caseload.

Key Responsibilities:

  • Develop case management plans that incorporate goals for both adults and children related to core goal areas, including finding stable housing and family stabilization and support.
  • Collaborate with families and Housing Specialists to assist with the housing search and placement process, including accompanying families to view units and advocating for families with landlords.
  • Network with community resources for housing, employment, substance abuse treatment, mental health treatment, training, and other services, and make referrals as appropriate.
  • Meet regularly with Youth Specialists to ensure children's needs are part of the parent's case management plan.
  • Obtain signed release of information forms and communicate regularly with staff at treatment programs, substance abuse programs, and vocational programs.

Requirements:

  • Bachelor's degree in social work or a related field required or 4 years of social service experience.
  • Minimum one year of work experience in social service or a related field preferred.
  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly preferred.
  • A valid driver's license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
