About the Role

We are seeking a dedicated and compassionate Housing Stability Case Manager to join our team at Community of Hope. As a key member of our staff, you will play a vital role in providing housing stability and income-focused case management services to families in need.

Key Responsibilities
  • Develop and implement case management plans that address the unique needs of each family, focusing on stable housing, income growth, and family stabilization.
  • Work closely with families to identify and access community resources, including housing, employment, and social services.
  • Collaborate with our Housing Specialist to facilitate housing searches and placements, and provide support to families throughout the process.
  • Network with community partners to ensure families receive comprehensive support and services.
  • Complete required assessments and documentation to track progress and outcomes.
Requirements
  • Bachelor's degree in social work or a related field, or 4 years of social service experience.
  • Minimum one year of work experience in social services or a related field, with a focus on homelessness prevention and family support.
  • Strong interpersonal, conflict-resolution, and organizational skills, with the ability to work independently and as part of a team.
  • A valid driver's license and regular access to a car, with proof of auto insurance.
