About the Role

We are seeking a dedicated and compassionate Case Manager to join our team at Community of Hope. As a Housing Stability Case Manager, you will play a vital role in supporting individuals and families in achieving housing stability and overall well-being.

Key Responsibilities
  • Develop and implement case management plans to support clients in achieving housing stability, increasing income, and family stabilization and support.
  • Meet with clients to review progress, address barriers to accessing services, and provide guidance and support.
  • Collaborate with clients and Housing Specialists to assist with housing search and placement, and advocate for clients with landlords and other stakeholders.
  • Network with community resources to connect clients with housing, employment, emotional wellness services, and other support services.
  • Maintain regular communication with clients and document progressive engagement efforts.
Requirements
  • Associate's degree or two years of college, or equivalent experience in homeless services or social services.
  • Minimum of two years of experience working in housing/homelessness or social services field.
  • Strong knowledge of community resources and ability to become knowledgeable required.
  • Strong interpersonal and organization skills required.
  • Strong communication skills, including verbal and writing.
  • Proven ability to submit timely, detailed, and accurate case notes.
