As a Care Coordinator, you will engage in various administrative and customer service tasks within the physician's office.

Key Responsibilities:

  • Register new patients in the electronic medical record (EMR) system, ensuring accurate demographic and insurance details.
  • Prepare essential documents such as initial consultations, notes, lab slips, and reports for physician review before patient appointments.
  • Manage patient records by opening chart encounters in the system, providing historical context.
  • Welcome and assist patients during their clinic visits.
  • Measure and record vital signs for physician assessment.
  • Input orders and e-prescriptions, and generate clinical visit summaries.
  • Coordinate follow-up appointments as necessary.
  • Maintain communication with the physician's team to ensure seamless patient care.
  • Conduct inventory checks and restock clinic supplies as needed.
  • Assist in daily clinic operations with minimal supervision.
  • Support Lead Care Coordinators and Practice Operations Supervisors with various tasks.
  • Manage recall lists, create diagnostic reviews, and follow up on outstanding orders.
  • Handle incoming calls, record messages, and schedule appointments for consultations and tests.
  • Uphold patient confidentiality and adhere to HIPAA regulations.
  • Ensure office equipment is functioning properly and coordinate repairs as needed.
  • May assist with surgery scheduling tasks as directed.
  • Provide coverage for other roles as required.
  • Participate in compliance training and adhere to guidelines.
  • Act as a representative of the practice, delivering outstanding customer service to patients and visitors.
  • Perform additional duties as assigned.

Qualifications:

  • High School Diploma or GED required; Bachelor's Degree preferred.
  • 1-2 years of experience in a medical environment is essential.
  • Exceptional verbal and written communication skills.
  • Strong organizational abilities and prioritization skills.
  • Dedication to providing an excellent patient experience and fostering a culture of excellence.
