Position Summary:


The role serves as a vital resource for individuals, their support networks, and the community services team in developing and sustaining housing services and assistance.

This position is responsible for ensuring the provision and financial management of supports as detailed in the individual's service plan.

Collaboration with the fiscal intermediary, community habilitation, finance, and accounting teams is essential to guarantee accurate billing and payment for services rendered.

Effective communication and liaison with external providers, regulatory bodies, individuals, and their families are crucial to maintain stability and continuity of housing supports and community habilitation services for those in customized residential settings.

Compliance with relevant regulations and agency policies related to housing and self-direction is a key responsibility, ensuring that updates are made as necessary to align with OPWDD guidelines.

Compensation:
$19.36 / hr - $22.58 / hr


The salary for the selected candidate will be determined based on various factors, including departmental budgets, qualifications, experience, education, licenses, specialties, and training.

The stated range reflects the organization's good faith estimate of potential compensation at the time of posting.

Key Responsibilities:


Promote the philosophy of self-direction through active engagement and alignment of departmental outcomes with the agency's strategic objectives.

Assist individuals, their support teams, and agency staff in person-centered planning and decision-making.

Build relationships with individuals, their families, and support staff grounded in mutual respect and empathy.

Facilitate real-life problem-solving and conflict resolution for individuals managing their own supports.


Oversee and manage job-related tasks, including: Documentation and billing in compliance with regulations and OPWDD Administrative Memorandums concerning self-directed services.

Conduct compliance reviews as outlined in the agency's compliance framework.

Monitor compliance inquiries for the Self-Direction team and ensure timely follow-up.

Manage payroll and HR functions for self-hired staff and Fiscal Intermediary Coordinators.

Provide education and training for self-hired employees.

Offer education, training, supervision, and leadership development for members of the Fiscal Intermediary team.

Oversee departmental finances and make necessary adjustments to maintain financial health.

Support the Self-Direction Intake Team with guidance and oversight.

Assist HCS Agency Brokers and provide training to Independent Brokers collaborating with Heritage Christian Services.

Supervise Associate Directors of Self-Directed Services and provide oversight for ISS/CTS transition stipend requests.

Maintain up-to-date knowledge of the OPWDD service system, particularly regarding Self-Direction.

Ensure that Self-Direction Policies and Procedures are regularly reviewed and updated as needed.

Act as a Fiscal Intermediary Coordinator for individuals when required.


Serve as the primary support for individuals in Customized Residential Options, including responsibilities such as: Acting as the main contact for families, other service providers, and the Community Habilitation management team.


Under the guidance of the Associate Director and/or Director of Customized Support, supervise and schedule the work of Community Habilitation staff to ensure adequate coverage based on individual needs.

Provide ongoing training for staff.

Document service delivery and assist colleagues with timely documentation.

Supervise the facilitation and documentation of each individual's services and monthly summaries.

Report all incidents, illnesses, or unusual circumstances to the Associate Director or Director of Customized Support.


Address maintenance concerns in the home, including general housekeeping and upkeep of grounds and equipment.

Train co-workers and model safe practices in the home, including fire safety, medical emergencies, and personal safety.

Qualifications:


Experience in a human services field is required, with a minimum of two years working with individuals with intellectual/developmental disabilities.

A Qualified Intellectual Disability Professional (QIDP) designation is preferred; however, a high school diploma is the minimum requirement.

Demonstrated excellence in teaching and developing strategies for individuals with intellectual disabilities is essential.

Evidence of supervisory and administrative capabilities is necessary.

A valid New York state driver's license that meets Heritage Christian Services, Inc. motor vehicle record standards is required.
