Responsibilities:
Key Duties:


Formulates the comprehensive Residential Service Plan in collaboration with the resident and their support network by identifying individual needs and aspirations to maintain housing stability and/or progress towards more independent living.


Deliver educational, monitoring, and recovery-oriented support in accordance with relevant regulations, including rehabilitative services such as Assertiveness Training, Community Integration, Daily Living Skills, Health Services, Medication Management, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.


Ensure each resident receives thorough planning in a person-centered manner for admission (including completion of admission documentation), ongoing recovery services, and discharge planning.

Guarantee that each plan is comprehensive and current in line with professional standards and the New York State Office of Mental Health guidelines.


Engage in monthly meetings with the Primary Counselor and Resident to assess recovery efforts, progress, and necessary adjustments to plans, ensuring service delivery aligns with each resident's service plan.


Conduct at least quarterly evaluations of the Residential Service Plan with the individual to determine progress and make necessary revisions.


Provide oversight and guidance to Counselors to ensure they observe, document, and report resident behaviors and progress in accordance with agency standards.


Assist Counselors in scheduling annual appointments for physicals, eye exams, and dental visits, ensuring transportation is arranged when necessary.


Collaborate with the resident's community service providers to ensure seamless continuity of care (including care managers, clinical, medical, educational, vocational, and entitlement specialists). Make appropriate referrals to these services as needed.


Act as the primary liaison between the resident and their clinical provider regarding the monitoring of the individual's mental health and overall well-being. Work alongside the Primary Counselor to keep the resident engaged in clinical services through regular attendance.


Assist residents in planning for the purchase of clothing and other essentials.


Encourage residents to foster healthy connections with family and community, as well as to explore new supports and interests.

Maintain resident confidentiality and demonstrate a caring, positive attitude towards all residents, staff, guests, and service providers.


Ensure safe transportation of residents when required.


Adhere to all safety protocols and regulations for self, residents, and staff.


Perform any additional duties necessary for the effective operation of the program.



Qualifications:

Education & Experience:

QMHS Standard Bachelor's degree in a Human Services field or an Associate's degree in a Human Services field plus three years of full-time related experience, or a High School Diploma plus five years of full-time Human Services experience.

A clean NYS driving record is also required as per DePaul's personnel policy. Candidates must be at least eighteen (18) years of age.

Work Environment:
We offer two scheduling options, both with varied shifts (mix of days and evenings):

Sunday - Thursday

Tuesday - Saturday
