Job Summary

We are seeking a highly skilled and compassionate Residential Services Coordinator I to join our team at DePaul. This role is responsible for developing and implementing individualized Residential Service Plans, providing teaching, monitoring, and recovery-oriented supports to residents, and ensuring continuity of care through collaboration with community service providers.

Key Responsibilities
  • Develop and implement individualized Residential Service Plans with residents and their support networks, identifying needs and desires to maintain placement and progress to more independent housing.
  • Provide teaching, monitoring, and recovery-oriented supports in accordance with OMH 595 and 593, including assertiveness/self-advocacy training, community integration/resource development, daily living skills, health services, medication management/training, parenting training, rehabilitative counseling, skill development, socialization, substance abuse services, and symptom management.
  • Ensure comprehensive planning in a person-centered approach for admission, on-going recovery-oriented services, and discharge, with each plan complete and up-to-date in accordance with standards of practice and the New York State Office of Mental Health.
  • Participate in monthly meetings with Primary Counselors and residents to monitor recovery efforts, progress, and changes needed in plans, ensuring delivery of services in accordance with each resident's service plan.
  • Conduct quarterly reviews of the Residential Service Plan with individuals to ascertain progress and make revisions as necessary.
  • Provide guidance and oversight to Counselors to ensure they observe, monitor, document, and report resident behaviors and progress in accordance with agency standards.
  • Collaborate with residents' community service providers to ensure continuity of care, making referrals as needed.
  • Serve as the key liaison between residents and their clinical providers, monitoring mental health and well-being, and working with Primary Counselors to keep residents engaged in clinical services.
  • Support residents in planning for essential needs, such as clothing and necessities, and encourage healthy connections with family and community ties.
  • Maintain resident confidentiality and demonstrate a caring, positive attitude toward all residents, staff, guests, and service providers.
  • Perform other duties necessary for the effective operation of the program.
Requirements
  • Education: Bachelor's degree in a Human Service field or an Associate's degree in a Human Service field plus three years of full-time related experience, or a High School Diploma plus five years of full-time Human Service experience.
  • Experience: Clean NYS driving record, as outlined in DePaul's personnel policy, and at least 18 years of age.
Work Environment

We offer two schedule options with varied shifts (mix of days and evenings): Sunday-Thursday and Tuesday-Saturday.
