POSITION SUMMARY
The Case Manager plays a vital role in the development and execution of Individualized Service Plans tailored to the unique needs of individuals and their advocates. This position operates under general supervision, ensuring compliance with relevant regulations and standards within a residential context. The Case Manager is responsible for assessing individual needs, preferences, and interests, while effectively linking clients to appropriate service providers.

KEY RESPONSIBILITIES
The responsibilities of the Case Manager encompass, but are not limited to:

  • Delivering social services aimed at enhancing the social functioning of individuals, supporting their families, and safeguarding their rights and dignity.
  • Creating and overseeing individualized treatment plans in alignment with regulatory guidelines and agency policies.
  • Identifying suitable service providers and facilitating connections to ensure comprehensive support.
  • Coordinating services and evaluating client satisfaction to enhance service delivery.
