We are seeking a skilled and compassionate Clinical Nurse Manager to lead our team at Community of Hope in Washington, DC. As the clinical lead, you will be responsible for ensuring high-quality patient care, smooth patient flow, and team coordination while following evidence-based guidelines.

About the Role
  • Lead the development of patient-centered individual care plans, coordinating care with patients across healthcare providers, settings, conditions, community service providers, and caregivers.
  • Supervise nursing/medical assistants at designated sites, providing guidance and support to ensure excellent patient care.
  • Assist with chronic care management by providing education and care to patients.
Responsibilities
  1. Ensure high-quality patient care, smooth patient flow, and team coordination, adhering to evidence-based guidelines.
  2. Develop and implement individualized care plans, incorporating patient preferences and needs.
  3. Serve as a resource for nursing/medical assistants, providing guidance and support to ensure exceptional patient care.
  4. Collaborate with multidisciplinary teams to address complex patient cases and develop innovative solutions.
Requirements
  • Bachelor of Science degree in Nursing (BSN) required.
  • Current, unencumbered DC Registered Nurse license with current CPR certification required.
  • At least one year of supervisory experience in an ambulatory, primary care, or med/surg setting.
  • Prior experience working with patients with substance use disorders and behavioral health concerns preferred.
  • Knowledge of chronic disease processes and health maintenance essential.
