Position Overview

The Hepatitis C Care Coordination Specialist is a dedicated regional role focused on providing essential care referrals for individuals living with Hepatitis C (HCV). This position is crucial in assisting clients in navigating resources for financial support, laboratory services, medications, and patient assistance programs related to Hepatitis C treatment. The Specialist will be primarily based at Cabarrus Health Alliance, with responsibilities extending across multiple counties, including Stanly, Union, Rowan, Mecklenburg, Gaston, Cleveland, Lincoln, Catawba, Alexander, and Iredell.

Key Responsibilities:

  • Act as a primary coordinator to connect individuals diagnosed with Hepatitis C to appropriate clinical providers for necessary care and treatment.
  • Facilitate prompt disease intervention by engaging patients swiftly, serving as the initial point of contact for newly diagnosed individuals or those not currently receiving care.
  • Deliver disease-specific education and support to clients.
  • Conduct on-site Hepatitis C testing in collaboration with local health departments and community partners.
  • Perform contact tracing for acute Hepatitis C cases utilizing the state surveillance system.

Qualifications: Bilingual proficiency in English and Spanish is preferred. A high school diploma or equivalent is required. Relevant experience in peer support, human services, communicable disease management, harm reduction, or care coordination is essential. A phlebotomy certification will be required and can be obtained post-hire.

Licensure: A valid driver's license in North Carolina is necessary.

Skills and Competencies:

  • In-depth knowledge of public health principles, practices, and harm reduction strategies.
  • Strong understanding of Hepatitis C, its treatment options, and effective care management techniques.
  • Ability to engage effectively with diverse populations, including individuals with active substance use disorders.
  • Familiarity with community resources and organizations available to support clients.
  • Capacity to collaborate with community partners and patients, both in-person and virtually.
  • Comfort in discussing sensitive topics related to social, sexual, and substance use issues in a non-judgmental manner.
