The Hepatitis C Care Coordinator is a specialized role focused on providing essential care referrals for individuals living with Hepatitis C (HCV). This position is based at Cabarrus Health Alliance and is responsible for facilitating access to care, managing patient connections, conducting HCV testing, and providing educational resources across multiple counties including Stanly, Union, Rowan, Mecklenburg, Gaston, Cleveland, Lincoln, Catawba, Alexander, and Iredell.
Key Responsibilities:
- Act as a liaison to connect individuals diagnosed with Hepatitis C to appropriate clinical providers for treatment.
- Engage promptly with patients to facilitate rapid disease intervention, serving as the initial contact for newly diagnosed individuals or those not currently receiving care.
- Deliver disease-specific education and support to clients.
- Conduct on-site Hepatitis C testing in collaboration with local health departments and community partners.
- Perform contact tracing for acute Hepatitis C cases utilizing the state surveillance system.
Qualifications: Bilingual proficiency in English and Spanish is preferred. A high school diploma or equivalent is required. Experience in peer support, human services, communicable diseases, harm reduction, and/or care management is essential. A phlebotomy certification is required and can be obtained post-hire.
Licensure: A valid North Carolina driver's license is necessary.
Skills and Competencies:
- In-depth knowledge of public health principles and harm reduction strategies.
- Strong understanding of HCV and its treatment options, with expertise in care management.
- Ability to engage effectively with diverse populations, including those with active substance use disorders.
- Extensive knowledge of community resources and organizations.
- Capacity to collaborate with community partners and patients both in-person and virtually.
- Comfort in discussing sensitive topics related to social, sexual, and substance use issues in a non-judgmental manner.
