Overview

Shift: Part-Time Weekend Days, 10 hours per day every weekend

This position involves a blend of triage responsibilities and home visits, allowing the team member to work remotely, with a preference for candidates with triage experience.

Job Summary

Delivers professional nursing care through proficient assessment, diagnosis, outcome identification, planning, implementation, and evaluation in alignment with service line policies. The Clinical Care Nurse will contribute to maintaining a secure work environment and perform all related job duties safely. The Clinical Care Nurse upholds clinical and professional competency appropriate to the age, culture, developmental stages, and specific needs of the patients served.

Essential Functions
  • Gathers data relevant to the healthcare consumer's health or situation.
  • Evaluates the assessment data to establish diagnoses or issues.
  • Identifies anticipated outcomes for a plan tailored to the healthcare consumer or situation.
  • Formulates a plan that outlines strategies and alternatives to achieve expected outcomes.
  • Executes the identified plan, coordinates care delivery, and implements strategies to promote health and a safe environment.
  • Assesses progress towards achieving outcomes.
  • Delegates aspects of care to suitable healthcare workers in accordance with applicable legal or policy parameters.

Physical Requirements
The role requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be capable of lifting and supporting the weight of 35 pounds when handling patients, medical equipment, and supplies. Proficiency in English is necessary for clear communication. Must possess intact senses of sight, hearing, smell, and touch, along with finger dexterity. Critical thinking and the ability to concentrate are essential. Must be able to respond promptly to changes in patient and/or unit conditions. Physical abilities testing may be required. Additional department-specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

Education, Experience and Certifications

Must be a graduate from an accredited School of Nursing. A Bachelor of Science in Nursing (BSN) is preferred. Basic Life Support (BLS) certification is required per policy guidelines. A current RN license or temporary license as a Registered Nurse Petitioner in the state of employment and residence is necessary; if declaring a National License Compact (NLC) state as the primary state of residency, the licensure requirements in the home state must be met; or for Non-National License Compact states, a current RN license or temporary license as a Registered Nurse Petitioner is required in the state where the RN practices. Additional education, training, certifications, or experience may be required within the department by the nurse leader.
