Partners Behavioral Health Management is seeking a highly skilled Care Manager to join our team. As a Care Manager, you will be responsible for providing comprehensive care management services to individuals with Intellectual and Developmental Disabilities (I/DD).
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to address the unique needs of each member.
- Provide education and guidance on self-management and self-advocacy to promote independence and empowerment.
- Facilitate access to and coordination of services, including healthcare, social services, and community-based resources.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
- Document all interactions and interventions in the member's electronic health record.
- Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
- Two years of full-time experience working with the I/DD population, or equivalent combination of education and experience.
- Ability to travel regularly to perform job duties.
- Must reside in North Carolina.
- Experience working with members who have co-occurring physical health and/or behavioral health needs.
