Vocational Rehabilitation Case Manager

Position Summary:
The Vocational Rehabilitation Case Manager is responsible for delivering comprehensive vocational rehabilitation services and effective case management to clients within their designated caseload. This role involves collaboration with clients, community resources, medical professionals, and employers to facilitate the client's successful return to meaningful employment.
Key Responsibilities:
- Conduct thorough career evaluations to assess vocational strengths and determine suitable employment fields for clients.
- Develop individualized vocational plans and coordinate with employers to support clients in their return-to-work journey.
- Identify and address barriers to employment, assisting clients in overcoming challenges to achieve long-term career goals.
- Provide counseling and support to clients regarding mental and physical health issues that may impact job retention.
- Engage with program participants, referral sources, and community resources to create effective vocational rehabilitation strategies.
- Ensure clients receive necessary services and participate in treatment planning with the multidisciplinary team.
- Maintain accurate client records in compliance with agency and regulatory standards.
- Participate in ongoing training, staff meetings, and performance improvement initiatives.
Qualifications:
General Requirements:
Candidates must demonstrate a strong understanding of addiction as a disease and adhere to ethical standards in the field. A background in case management, social work, or related fields is preferred, along with experience working with diverse populations.
Technical Skills:
Proficiency in Microsoft Office and familiarity with job search platforms are essential for assisting clients with resume writing and interview preparation.
Educational Background:
An Associate Degree or significant progress towards one in a relevant field such as Rehabilitation Counseling or Social Work is preferred.
Certification:
CDCA certification is required, with CRC certification or equivalent preferred.
Travel Requirements:
Minimal to moderate travel may be necessary, primarily local, utilizing agency vehicles as needed.
Work Schedule:
This is a full-time position, typically requiring 40 hours per week, Monday through Friday.
Compensation:
Salary will be determined based on licensure and experience.
