Job Overview

Position Summary:

The Case Manager serves as the main advocate for clients, ensuring continuous monitoring of their progress while orchestrating all services provided both internally and through external referrals.

Key Responsibilities:

  1. Guaranteeing that clients receive all necessary services.
  2. Facilitating coordination of services from ancillary organizations, detox centers, and hospitals.
  3. Engaging in treatment planning and reviews alongside the entire treatment team.
  4. Consulting with clients regarding the achievement of treatment plan goals and objectives.
  5. Conducting comprehensive needs assessments and formulating service plans.
  6. Delivering individual and group counseling focused on case management and life skills.
  7. Collaborating on the development of discharge plans with other treatment team members.
  8. Working with the primary therapist to ensure the execution of behavioral management strategies.
  9. Maintaining client records in accordance with agency, state, and local regulations.
  10. Participating in training sessions, staff meetings, and supervisory discussions.
  11. Engaging in performance improvement initiatives.
  12. Performing additional duties as assigned.

Qualifications:

General Requirements:

Candidates must comply with the Chemical Dependency Code of Ethics established by the Ohio Chemical Dependency Professionals Board, possess a solid understanding of addiction as a disease, and have a background in case management, social work, or relevant coursework in chemical dependency. Experience working with specific populations in community or similar settings is preferred. Strong written and verbal communication skills are essential, along with the ability to effectively collaborate with agency personnel, external contacts, and a diverse client base. A valid Driver's License and insurability through the agency's automobile insurance are required.

Educational Background:

An Associate Degree or significant progress towards an Associate Degree with a focus on Addictions Treatment and Prevention or equivalent is preferred.

Certification:

CDCA certification is mandatory.

Travel Requirements:

Minimal travel, primarily local.

Work Schedule:

Full-time, Monday through Friday (Day Shift), totaling 40 hours per week.

Compensation:

Salary will be commensurate with licensure and experience.
