Job Description

Position: Case Manager (FLOATER)

Employment Type: Full-Time

Salary: $40,000/Year

SCHEDULE: Full-time, determined on a case-by-case basis

OVERVIEW: This role involves serving as a community-based case manager dedicated to assisting homeless and at-risk veterans facing housing challenges.

KEY RESPONSIBILITIES:

  1. Conduct outreach in community settings where homeless and at-risk veterans gather to ensure effective engagement.
  2. Evaluate individual needs, risks, and barriers to achieving household stability and securing long-term housing.
  3. Facilitate financial assistance for homelessness prevention or rapid re-housing services as required.
  4. Ensure that participant housing meets established organizational standards for habitability.
  5. Coordinate and connect clients with community resources tailored to their specific needs.
  6. Assist participants in formulating realistic and actionable housing plans aimed at achieving stability, complete with measurable steps and timelines.
  7. Maintain consistent communication with households, including assessments related to childcare, crisis intervention, referrals, and ongoing life skills development.
  8. Monitor progress towards established goals, adjusting individual service plans as necessary.
  9. Document all significant interventions, including diagnostic steps, progress reports, and utilization of referral services.
  10. Gather and submit required data for the HMIS database.
  11. Actively engage in regular team meetings to ensure program consistency and effectiveness.
  12. Provide crisis intervention when required.
  13. Implement Housing First and Motivational Interviewing best practices in service delivery.
  14. Participate in community events that promote services for veterans.
  15. Adhere to agency policies, COA regulations, and federal and state requirements, including educational and certification standards.
  16. Perform additional duties as assigned.

QUALIFICATIONS:

  1. Strong organizational, interpersonal, and communication skills.
  2. Willingness to learn and apply best practice models as needed.
  3. Valid Ohio Driver's License and ability to meet agency driving insurability requirements; reliable transportation.
  4. Ability to pass a criminal background check as per agency requirements.

EDUCATIONAL REQUIREMENTS: Preferred college-level coursework in human services or a related field.

EXPERIENCE REQUIREMENTS: Preferred experience in addressing issues related to homelessness, domestic violence, substance abuse, and mental health, with a focus on working with veterans.
