Position: Case Manager (FLOATER)

Salary: $40,000/Year
SCHEDULE: Full-time, determined on a case-by-case basis
OVERVIEW: This role involves serving as a community-focused case manager dedicated to assisting homeless and at-risk veterans facing housing challenges.
KEY RESPONSIBILITIES:
- Conduct outreach in community settings where homeless and at-risk veterans gather to ensure effective engagement.
- Evaluate individual needs, risks, and barriers to achieving stable housing and long-term solutions.
- Facilitate financial support for homelessness prevention or rapid re-housing services as required.
- Ensure that participant housing meets established organizational standards for habitability.
- Connect and coordinate community resources tailored to individual requirements.
- Guide participants in formulating realistic housing plans that emphasize stability, including actionable steps and timelines.
- Maintain ongoing communication with households, providing assessments, crisis intervention, referrals, and life skills development.
- Monitor progress towards goals, adjusting individual service plans as necessary.
- Document all significant interactions, diagnostic steps, progress updates, and utilization of referral services.
- Gather and submit necessary data for the HMIS database.
- Engage actively in regular team meetings to ensure program consistency and effectiveness.
- Provide crisis intervention as needed.
- Implement Housing First and Motivational Interviewing best practices in service delivery.
- Participate in community events to promote services available for veterans.
- Adhere to agency policies, COA regulations, and federal and state requirements.
- Perform additional duties as assigned.
QUALIFICATIONS:
- Strong organizational, interpersonal, and communication skills.
- Willingness to learn and apply best practice models as appropriate.
- Valid Ohio Driver's License and ability to meet agency driving insurability requirements; reliable transportation.
- Must meet agency requirements for criminal background checks.
EDUCATIONAL REQUIREMENTS: Preferred college-level coursework in human services or a related field.
EXPERIENCE REQUIREMENTS: Experience with homelessness, domestic violence, substance abuse, and mental health issues preferred. Experience working with veterans is also preferred.
