Position Overview
The role of the Care Management Manager encompasses a variety of responsibilities, primarily focusing on the assessment, planning, direction, implementation, and evaluation of all activities within the designated department.

Key Responsibilities
- Oversee the hiring, orientation, evaluation, and supervision of care management and social work personnel.
- Organize schedules, assess caseloads, and assign daily tasks.
- Identify and address educational needs of staff, manage performance improvement initiatives, and implement corrective actions as necessary.
- Contribute to the development of departmental budgets, policies, and procedures.
- Assist in creating practices for utilization review and discharge planning, ensuring adherence to managed care contracts and regulatory standards.
- Analyze productivity and outcome metrics to discover opportunities for ongoing enhancement.
- Manage information systems to optimize discharge planning efficiency.
- Establish relationships with post-acute care providers and serve as a clinical expert for interdisciplinary teams.
- Perform additional duties as assigned.

Qualifications
- Candidates must meet the educational requirements for either Nursing or Social Work.

For Registered Nurses:
- A Bachelor of Science in Nursing (BSN) or a related field is required; a Master's degree is preferred.

For Social Workers:
- A Master's degree in Social Work accredited by the Council on Social Work Education is required.

Certifications
- For Registered Nurses: A current and valid license to practice as a Registered Nurse (RN) in the state of employment is required; professional certification as a Case Manager (CCM) is preferred.
- For Social Workers: A current and valid license to practice as a Licensed Social Worker (LSW) in the state of employment is required.

Experience
- A minimum of three years of experience in utilization management, case management, or discharge planning, along with three years of clinical experience in a healthcare setting, is required. Supervisory experience in care management or social work is preferred.

Physical Requirements
- Ability to remain stationary for extended periods, normal or corrected vision, and manual dexterity to operate a personal computer are necessary. The role may require light physical activity, including lifting up to 20 pounds occasionally.

Personal Protective Equipment
- Adherence to standard precautions and use of personal protective equipment as required is essential.
