Your Role
The Care Management Manager plays a crucial role in overseeing the assessment, planning, direction, implementation, and evaluation of all activities within the assigned department. You will be instrumental in developing, coordinating, and assessing a program aimed at continuously enhancing the quality of care and services provided.

Key Responsibilities
Your responsibilities will encompass:

  • Assisting with the recruitment, orientation, evaluation, and supervision of care management and social work personnel.
  • Organizing schedules, assessing caseloads, and assigning daily tasks.
  • Identifying educational needs of staff and implementing performance improvement initiatives.
  • Providing insights into departmental budgeting, policies, and procedures.
  • Developing and refining practices for utilization review and discharge planning.
  • Analyzing productivity metrics to identify areas for continuous improvement.
  • Managing information systems to enhance discharge planning efficiency.
  • Establishing collaborative relationships with post-acute care providers.
  • Serving as a clinical expert for interdisciplinary teams.

Qualifications
To be considered for this position, candidates must meet the educational requirements for either Nursing or Social Work:

  • For Registered Nurses: A Bachelor of Science in Nursing (BSN) or a related field is required; a Master's degree is preferred.
  • For Social Workers: A Master's degree in Social Work accredited by the Council on Social Work Education is required.

Certifications
Registered Nurses must hold a current and valid license to practice in their state. Professional certification as a Case Manager (CCM) is preferred. Social Workers must also have a valid license to practice as a Licensed Social Worker (LSW).

Experience
A minimum of three years in utilization management, case management, or discharge planning is required, along with three years of clinical experience in a healthcare setting. Supervisory experience in care management or social work is preferred.

Physical Requirements
This role requires the ability to remain stationary for extended periods, normal or corrected vision, and manual dexterity for computer use. Light physical activity may be necessary.
