Job Title: Care CoordinatorUnison Behavioral Health Group: A Leader in Behavioral Health Services

Unison Behavioral Health Group is seeking experienced professionals to join our team as full-time Care Coordinators. As a Care Coordinator, you will work closely with children and youth with behavioral or developmental disabilities, as well as their families and community partners, to link them to necessary care and services.

Key Responsibilities:
  • Case Management: Develop and implement individualized care plans to meet the unique needs of each child and family.
  • Community Outreach: Establish and maintain relationships with community partners, including healthcare providers, schools, and social service agencies, to ensure seamless care coordination.
  • Family Support: Provide education, support, and advocacy to families to empower them to make informed decisions about their child's care.
  • Collaboration: Work closely with multidisciplinary teams, including healthcare providers, therapists, and educators, to ensure comprehensive care.
Requirements:
  • Education: High School Diploma and 3 years' experience, or Associates or Bachelor's degree and 2 years' experience, or Master's degree and 1 year experience.
  • Experience: Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
  • Skills: Expertise in one or more of the following areas: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
  • Professional Development: Valid driver's license, reliable transportation, and personal auto insurance.
  • Certifications: LSW/LPC preferred.
