Job Summary
Unison Behavioral Health Group is seeking a highly skilled and experienced Care Coordinator Supervisor to lead a team of care coordinators in the Ohio Rise Program. This is a full-time position responsible for promoting the recovery of children and families participating in the program.
Key Responsibilities
- Implement the required model of care, providing individualized care based on client need and linking clients to needed services and supports.
- Ensure assigned staff participate in ongoing training and coaching to ensure consistency in delivering care coordination.
- Monitor completion of paperwork by assigned staff, including initial assessments, CANS assessments, care plans, and discharges.
Requirements
- Bachelor's degree in social work, psychology, or a related field with a master's degree preferred.
- LSW, LPC, LISW, or LPCC preferred.
- Minimum three years' experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
- Must be proficient in the use of computers and software.
- Possess a valid driver's license, reliable transportation, and be able to be insured through the agency's commercial policy.
