Key Responsibilities
  • Assist in managing the patient navigator team to ensure all patient needs are met.
  • Collaborate with providers to monitor and assess patient panels, identifying individual and population-specific needs to optimize patient engagement and team productivity.
  • Develop comprehensive, collaborative care plans based on provider treatment plans, evidence-based guidelines, and patient/family goals to promote treatment acceptance and adherence.
  • Oversee population-specific risk management through registry and referrals.
  • Identify barriers to treatment goals and develop intervention approaches to improve data-driven outcomes.
  • Track program-specific and patient-level quality measures to inform care coordination strategies.
  • Serve as a supportive resource and community referral resource within the practice.
  • Utilize electronic health records to track navigation services, record patient encounters, and contribute to clinic tracking workflows.
Requirements
  • Bachelor's degree in public health, social work, or a related field (or equivalent experience).
  • Relevant work experience in patient education, nursing, or case management.
  • Exceptional writing and personal communication skills.
  • Ability to work independently, prioritize tasks, and meet established deadlines.
  • Experience working with multidisciplinary medical teams and knowledge of patient-centered medical home principles.
  • Proficiency in Microsoft Suite and electronic health records.
