Position Overview:
Unison Behavioral Health Group is in search of a dedicated Care Coordinator Supervisor to oversee a dynamic team within the Ohio Rise Program. This role is pivotal in fostering the recovery journey of children and families engaged in the program.

Key Responsibilities:
This position entails the supervision of direct service care coordinators across designated counties, ensuring the delivery of high-quality support and services. The Supervisor will:

  • Implement the established care model, tailoring services to meet individual client needs.
  • Facilitate ongoing training and mentorship for staff to maintain consistency in care coordination.
  • Oversee the completion of essential documentation by team members, including initial assessments, CANS assessments, care plans, and discharge summaries.

Qualifications:
The ideal candidate will possess:

  • A bachelor's degree in social work, psychology, or a related discipline; a master's degree is preferred.
  • Licensure such as LSW, LPC, LISW, or LPCC is advantageous.
  • A minimum of three years of experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a similar public sector human services field.
  • Proficiency in computer systems and software applications.
  • A valid driver's license and reliable transportation, with the ability to meet agency insurance requirements.
  • Prior supervisory experience is preferred.
