Overview:


The Clinical Care Coordinator is responsible for ensuring seamless coordination and continuity of care as patients transition between acute care settings and post-acute environments. This role involves providing education and discharge planning to maintain care continuity throughout these transitions.

In this position, you will engage with patients and their caregivers to conduct barrier assessments, propose solutions to enhance patient care, advocate for the identification of life goals, and contribute to the treatment planning process.

The Clinical Care Coordinator will also facilitate the communication of a patient's treatment plan and overall status to all healthcare providers and caregivers throughout the care continuum. This role includes assisting patients in understanding how to access appropriate care by connecting them with relevant resources, whether through phone, email, or in-person interactions.



Key Responsibilities:

Essential Duties Include:

  • Conduct daily post-discharge follow-up calls to meet billing requirements.
  • Perform weekly 'Care Calls' to at-risk patients as directed.
  • Document all interactions by entering data into a computerized database.
  • Anticipate customer needs and proactively assist as necessary.
  • Operate computerized telephone systems effectively.
  • Compile, maintain, and process records, data, documentation, and reports regarding calls and actions taken.
  • Assist with research to identify solutions to customer-related issues.
  • Ensure customer satisfaction and document calls appropriately, following up as needed.
  • Initiate problem-solving and pursue continuous improvement in customer satisfaction.
  • De-escalate situations with upset callers by fostering a partnership with the customer.
  • Maintain the highest level of confidentiality.
  • Demonstrate the ability to multitask and communicate clearly and professionally over the phone.
  • Serve as an educational resource regarding hospice and home care for providers, patients, and caregivers.
  • Educate patients and caregivers on the significance of continuity of care, enabling effective communication among providers and identifying care gaps.

This approach aims to reduce hospital readmissions, improve outcomes, and enhance patient satisfaction.



  • Be accessible via phone and email to partners, providers, peers, and supervisors during working hours, with flexibility in scheduling to meet patient and caregiver needs.
  • Travel to patient locations, such as hospitals and skilled nursing facilities, when necessary to assess patient needs and status.
  • Monitor the delivery of appropriate home care, hospice, and ancillary services as directed by the care team.
  • Contact high-risk patients post-discharge to mitigate the risk of rehospitalization.
  • Perform additional duties as assigned.

Required Knowledge, Skills & Abilities:

Minimum Qualifications:

  • Active LPN licensure in Ohio and Michigan with a minimum of 3 years of relevant nursing experience.
  • Experience working with diverse populations.
  • Exceptional problem-solving, critical-thinking, organizational, interpersonal, and communication skills.
  • Ability to work independently with attention to detail and follow-through.
  • Capacity to function effectively in a critical care environment.
  • Ability to prioritize tasks and respond to inquiries effectively.
  • Proficient in computer applications (email, Word, Excel, PowerPoint, etc.).
  • Ability to collaborate effectively with various departments and administrative levels.
  • Willingness to adhere to all organizational and departmental policies and procedures.
  • Consistent punctuality and professionalism in attire and task completion.
  • Proactive in seeking additional tasks when primary responsibilities are fulfilled and assisting colleagues as needed.
