Key Responsibilities:

  • Conduct and Coordinate Resident Assessments: Develop and implement procedures to ensure timely and accurate completion of resident assessments, including the MDS.
  • Maintain and Update Policies and Procedures: Develop and maintain written policies and procedures that implement MDS and care plan requirements.
  • Assist Residents with Care Plans: Assist residents in completing the care plan portion of their discharge plan, ensuring that their needs and goals are met.
  • Collaborate with Healthcare Team: Work closely with the Director of Nursing Services and other healthcare team members to ensure seamless coordination of resident care.
  • Develop and Implement Procedures: Develop and implement procedures to inform all assessment team members of the arrival of newly admitted residents.
  • Assist with Scheduling: Assist Facility directors and supervisors in scheduling resident assessment and care plan meetings.
  • Participate in Quality Improvement: Participate in the development and implementation of resident assessments and care plans, including quarterly and annual reviews.

Requirements:

  • Current Ohio RN License: Must have a current and active Ohio RN license.
  • Long Term Care Experience: Must have experience working in long term care settings.
  • MDS Coordinator Experience: Must have experience as a MDS Coordinator.
  • MDS 3.0 Knowledge: Must have knowledge of MDS 3.0 requirements and regulations.
