About the Role

Franciscan Care Center is seeking a highly skilled MDS Coordinator to join our leadership team. As a key member of our clinical operations team, you will play a critical role in ensuring the accuracy and quality of our resident assessments and care plans.

Key Responsibilities
  • Resident Assessment and Care Planning: Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Accuracy and Timeliness: Ensure accurate and timely completion of the MDS process to meet regulatory requirements.
  • Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans that meet their unique needs and goals.
  • Progress Monitoring and Documentation: Monitor and document resident progress and responses to care plans to inform ongoing care and quality improvement initiatives.
  • Quality Assurance and Performance Improvement: Participate in the Quality Assurance and Performance Improvement (QAPI) process to identify areas for improvement and implement evidence-based solutions.
  • Communication and Coordination: Communicate effectively with residents, families, and other healthcare providers to coordinate optimal care delivery and ensure seamless transitions.
  • Regulatory Compliance: Stay up-to-date on MDS regulations and guidelines to ensure compliance and maintain the highest standards of care.
Requirements
  • Clinical Expertise: Current RN (Registered Nurse) license and RAC-CT (Resident Assessment Coordinator-Certified) certification.
  • Leadership and Collaboration: Strong understanding of MDS 3.0, RAI process, and care planning, with excellent organizational, communication, and interpersonal skills.
  • Quality Focus: Commitment to providing high-quality care to residents and contributing to a culture of excellence within the organization.
