Job Overview

Weinberg Campus is seeking a highly skilled and compassionate Senior Case Manager to join our team. As a Senior Case Manager, you will be responsible for providing support to 13 seniors in an Office of Mental Health group home.

Key Responsibilities:

  • Teach residents problem-solving techniques and assist them in developing skills in all areas of functioning, including health, medication, symptom, nutrition, budget, and home management.
  • Link residents with benefits, external services, and community resources.
  • Track, coordinate, and follow up with residents' medical and psychiatric needs, including accompanying residents to appointments and advocating for their needs.
  • Engage residents in formulating resident service plans and identifying goals that meet their aspirations and needs.
  • Write plans that are behavioral in nature, clearly stated, and reflect principles of rehabilitation.
  • Meet with residents on a regular basis to monitor performance and develop new objectives and activities.
  • Effectively respond to on-call issues.

Requirements:

  • Bachelor's degree in a Human Service-related field.
  • Valid New York State Driver's License.
  • At least 1 year of experience working in Human Services or Mental Health Services.
