Position Overview

We are seeking a dedicated Part-Time Housing Stability Case Manager/Data Manager to support our mission in providing essential services to individuals experiencing homelessness.

POSITION SCOPE AND OBJECTIVES:
The Housing Stability Case Manager/Data Manager will play a crucial role in assisting clients to swiftly transition out of homelessness and secure stable housing. This outreach position focuses on families at various stages of housing relocation, utilizing a Housing First approach to create tailored, person-centered housing stability support plans.

Key Responsibilities:
1. Identify and research affordable housing options for individuals facing homelessness and engage landlords to create housing opportunities for participants.
2. Educate landlords on the benefits of the Rapid Rehousing program and address any potential barriers to their participation.
3. Maintain a comprehensive understanding of the rental market, including Housing Quality Standards and Fair Market Rents.
4. Conduct housing needs assessments and determine participant eligibility for the program.
5. Provide direct case management or coordinate with other case managers to ensure continuity of services.
6. Foster effective communication with local community organizations and external agencies to facilitate access to necessary resources.
7. Assist participants in locating suitable rental housing that aligns with their needs and financial capabilities.
8. Support participants in negotiating reasonable lease agreements and developing skills for successful tenancy.
9. Maintain organized and up-to-date lease and compliance files, along with all client case documentation.
10. Engage in ongoing outreach and creative engagement strategies with tenants.
11. Conduct thorough assessments and assist tenants in formulating actionable plans to achieve their goals.
12. Collaborate with tenants and property management to prevent evictions and establish housing permanency plans.
13. Provide support in developing essential life skills, including understanding tenant rights and responsibilities.
14. Connect tenants with employment opportunities and access to medical, mental health, and psychosocial support services as needed.
15. Offer crisis intervention when necessary.
16. Ensure compliance with documentation requirements as mandated by program needs and HUD regulations.
17. Maintain confidentiality and respect client privacy while promoting their independence.
18. Collaborate with the Development Department to assist in grant writing and fundraising efforts for the program.
19. Input all relevant data into the Homeless Management Information System and ensure accurate reporting.
20. Participate in ongoing training and staff meetings as required.
